The Self–Determination Program (SDP) empowers individuals with intellectual and developmental disabilities and their families to take an active role in shaping their future. By offering greater choice, flexibility, and control over services and supports, SDP allows participants to create personalized plans that reflect their strengths, interests, and unique goals.
At Anthesis, we are proud to support individuals participating in the Self–Determination Program by providing personalized, person-centered services that promote independence, community inclusion, and meaningful opportunities. We collaborate with participants, families, and their support teams to deliver services that align with each individual’s Person-Centered Plan, honoring their choices while helping them build fulfilling, self-directed lives.
